Optomap Test Requirement:

• As part of our standard care, all patients are required to undergo an Optomap test.

• This retinal imaging captures detailed pictures of the back of the eyes, enabling our doctors to detect potentially life-threatening conditions, such as a stroke, heart disease, hypertension, and diabetes.

• Early signs of these conditions can show on the retina before any visual or other symptoms are revealed.

• Fortunately, most vision plans cover this test with only a $39 copay.

Appointment Guidelines:

• Please call our office to schedule multiple appointments, instead of using the online scheduler.

• A maximum of two members of one family may be scheduled on the same day.

• If under the age of 18, patient must be accompanied by a legal guardian.

Injuries or Emergencies:

• If you have an eye injury, sudden flashes, floaters, or vision loss, please call our office.
